General
Consequence of diseases with
chronic pleural scarring
Asbestos-related pleural disease
TB
Rounded Atelectasis
General
Asymptomatic
Resembles bronchogenic ca
Most often posteromedially at lung
bases
Subpleural in position
Rapidly forming pleural effusion
produces adjacent area of passive
atelectasis
Groove of visceral pleura may infold
into area of atelectasis and come to
surround part of it
Rounded Atelectasis
Pathophysiology
If effusion recedes at once
Lung will probably re-expand
If fibrinous adhesions form, or
If there is pre-existent chronic pleural
disease
Then atelectatic area of lung remains trapped
by enfolded visceral pleura

Rounded density at lung base
Contiguous to area of pleural disease or
superimposed on apparent asbestos-
related pleural disease or TB
May have air bronchogram

Comet tail
on CT
Vessels and bronchi converge upon and appear
to swirl around mass
Crow’s feet
— linear bands radiating from
mass into lung parenchyma
Linear densities radiate back toward hilum
